Medicare Advantage or Medigap? The questions that decide it
Most people choose between a Medicare Advantage plan and Original Medicare with a Medigap policy and a drug plan. Neither is better for everyone. These are the questions that decide it, and the ones to ask any agent who recommends one.

How they differ

- Original Medicare + Medigap: Any doctor or hospital in the U.S. that accepts Medicare
- Monthly cost
- Medicare Advantage: Varies; some plans have a $0 premium (plus your Part B premium)
- Original Medicare + Medigap: A Medigap premium and a drug plan premium (plus Part B)
- Cost when you use care
- Medicare Advantage: Copays and coinsurance, up to a yearly out-of-pocket limit
- Original Medicare + Medigap: Depends on the Medigap plan; many leave little to pay
- Drug coverage
- Medicare Advantage: Usually included
- Original Medicare + Medigap: A separate Part D plan
- Extras
- Medicare Advantage: Often dental, vision or hearing benefits
- Original Medicare + Medigap: Generally none; buy separately if you want them
- Referrals and approvals
- Medicare Advantage: Often needed for specialists and some services
- Original Medicare + Medigap: Usually not needed

- Original Medicare + Medigap: Medigap benefits by plan letter stay the same; premiums can rise
Questions that decide it
Are my doctors and hospitals in the network?
If keeping specific doctors matters most, check every one, for every plan, before choosing.Do I travel, or live part of the year in another state?
Networks are usually local. Original Medicare with Medigap works nationwide. What could I afford in a bad year?
Compare each plan's yearly out-of-pocket limit with what Medigap would leave you to pay.How are my prescriptions covered?
Check each drug, its tier and the pharmacies, in both the Advantage plan and the Part D plans. How much do extras matter to me?
Dental, vision and hearing benefits vary a lot. Ask what's actually covered, not just whether it's "included."
Switching later
This is the part people miss. Your first chance to buy Medigap, the 6 months after your Part B starts, is the one where insurers can't turn you down. If you choose Medicare Advantage first and want Medigap later, insurers in most states can ask health questions.
There are exceptions. If you join Medicare Advantage when you first become eligible at 65, or leave Medigap to try Medicare Advantage for the first time, federal "trial rights" generally let you switch back within the first 12 months and buy Medigap without health questions. Some states also give yearly rights; see the state-by-state list.
Common questions
Which is cheaper, Medicare Advantage or Medigap?
Some Medicare Advantage plans have $0 premiums, but what you pay when you get care varies, up to the plan's yearly limit. Medigap adds a premium but helps pay what Original Medicare doesn't. Which costs less overall depends on your health and the plans where you live.
Can I have both Medicare Advantage and Medigap?
No. Medigap only works with Original Medicare, and it's illegal for anyone to sell you a Medigap policy if they know you're in a Medicare Advantage plan (unless you're switching back to Original Medicare).
Can I switch from Medicare Advantage to Medigap later?
You can return to Original Medicare during set periods, but outside your guaranteed-issue rights, Medigap insurers in most states can ask health questions and turn you down.
Is an agent biased toward one or the other?
An agent may earn different amounts from different plans. Ask how many companies they represent and why they recommend what they do, and get a second opinion from your state's free SHIP if you're unsure.
